Job Description

Are you a compassionate, skilled social worker who thrives in high-impact moments of care?

We’re seeking a Licensed Master Social Worker (LMSW) to support General Inpatient (GIP) hospice patients across Dallas and Collin County —providing critical emotional, psychosocial, and crisis support to patients and families during some of life’s most vulnerable transitions.

This is more than a job—it’s an opportunity to be a steady, guiding presence when families need it most.

For nearly 100 years, the  Visiting Nurse Association of Texas (VNA) has been a trusted pioneer in hospice care, helping our neighbors age with dignity and independence. Our mission-driven work spans 16 counties across North Texas—including Collin, Dallas, Denton, Kaufman, and Tarrant to rural communities in Fannin and Navarro—ensuring no one faces life's transitions alone. As a non-profit, we prioritize people over profits, treating every patient like family and maintaining a gold standard of clinical excellence. Beyond hospice, our community roots run deep through programs like Meals on Wheels. We are looking for a Chaplain who thrives in a collaborative, IDT environment and is ready to carry forward our legacy of compassionate, high-quality spiritual care.

Joining our team means more than a competitive salary and mileage reimbursement. We offer comprehensive medical, dental, and vision plans, along with employer-paid life and short and long-term disability insurance. To support your future, we offer a 401(k) with a 6% match and a fast, two-year vesting schedule. We also prioritize work-life harmony through generous PTO, eight total paid holidays, and opportunities for professional growth and tuition reimbursement—all within a supportive, mission-driven culture.

Core Responsibilities

As our GIP-focused LMSW, you will:

  • Provide intensive psychosocial support to patients receiving General Inpatient hospice care
  • Lead crisis intervention efforts, helping families navigate urgent and emotional situations with clarity and compassion
  • Facilitate family meetings and mediation, ensuring patient-centered, goal-aligned care
  • Partner closely with hospital case managers, nursing teams, and interdisciplinary staff
  • Support discharge planning and care transitions, ensuring continuity and dignity in care
  • Deliver education and emotional support to caregivers and loved ones
  • Maintain timely, accurate documentation in alignment with hospice regulations

Requirements & Qualifications:

  • Education : Master’s Degree in Social Work (MSW) from a state-accredited program
  • Licensure : Current Texas Certification (LMSW)
  • Experience : At least one (1) year of social work experience in a health care agency.
  • Skills : Strong interpersonal skills and a deep understanding of community resources.
  • Compliance : Must pass criminal background checks and provide proof of TB testing and a Hepatitis profile/acceptance.
  • Transportation : A valid Texas Driver’s License, reliable transportation, and valid auto liability insurance for local travel to patient homes.

Working Conditions

This position involves traveling locally to visit patients in their homes. You should be comfortable working in various conditions, including occasional exposure to infectious diseases and unpleasant weather. The role requires prolonged standing, walking, and the ability to make quick decisions under stressful conditions to meet patient needs.

The Visiting Nurse Association of Texas is an Equal Opportunity Employer. We do not discriminate on the basis of race, color, religion, sex, national origin, age, disability, genetic information, or any other status protected by federal, state, or local law.
