Position Summary:

Reporting to our Assistant VP of Medical Affairs, the Clinical Pharmacist will join our team on a full time basis to provides medication management, medication adherence, medication reconciliation and medication education services. Our Clinical Pharmacist positively impacts our patient's ability to manage their chronic condition. 

Minimum Qualifications:

• Doctor of Pharmacy Degree required from ACPE accredited college. 
• NYS Licensed Pharmacist
• Postgraduate Year 1 (PGY1) and/or Postgraduate Year 2 (PGY2) in Ambulatory Care Residency preferred
• Bilingual Spanish/English highly desired. 

Key Duties:

• Takes Medical history of patient, medication reconciliation, and allergies.
• Provides patient education and counseling.
• Provides collaborative drug therapy management (CDTM) services.
• Maintains formulary, evaluates new formulary requests and maintains medication management policy and procedures.
• Provides tailor made therapeutic regimen, adjustment, and titration of doses for particular patients.
• Other clinical and administrative duties, as assigned. 

This position is a hybrid role, requiring onsite duties based in the Bronx, NY along with some remote/work-from-home capabilities. 

Compensation- $120,000-$125,000 contingent on experience based on 40 hours per week.

Schedule: Monday to Friday 8:00 am to 5:00 pm.
