Key Responsibilities Provide direct care to clients living outside Brookdale communities, promoting the client’s highest level of functioning. Assist in maintaining the client’s environment in a safe and neat manner. Perform personal care and/or concierge/homemaker/companion services per individualized plan of care. Assist clients with activities of daily living: bathing, dressing, grooming, toileting, transferring, meals. Document all pertinent information regarding care as assigned. Perform vital signs, BP, weight, and other trained procedures as per client’s care plan. Maintain a working knowledge of the home‑care agency’s policies and procedures. Communicate effectively with clients, families, visitors, and agency leadership. Maintain client confidentiality and adhere to HIPAA policies. Travel to client homes and arrive on time for assignments. Qualifications High school diploma or GED required. Proof of completing an approved Certified Nursing Assistant program. Six months of experience working independently with clients in their homes preferred. Successful completion of State CNA course and maintenance of certification. Valid state identification card and reliable transportation. Valid driver’s license preferred. Compensation & Benefits Base pay range determined by skills and experience.
