The Community Health Worker (CHW) is a trusted connection between the community and local health and social service systems. In this role, you’ll build meaningful relationships, help individuals navigate care, and empower community members to improve their health and well-being.
This position plays a key role in reducing health disparities by providing outreach, education, advocacy, and support. The CHW works closely with healthcare providers, public health programs, and community organizations to ensure individuals have access to the resources they need.
Serve as a liaison between community members and healthcare, public health, behavioral health, and social service systems
Conduct outreach and community engagement activities to promote health and wellness
Help individuals access services and resources, reducing barriers to care
Provide informal counseling, social support, and health education
Advocate for individuals and communities to improve access and quality of care
Identify community health needs and assist in developing strategies and resources to address them
Support and participate in community meetings, coalitions, workshops, and events
Collaborate with Project Coordinators and ECDHD programs
Maintain accurate documentation and follow program requirements
Participate in trainings and professional development opportunities
Perform other duties as assigned
Required:
High school diploma or equivalent
Willingness to obtain a Community Health Worker certification within one year of hire
Strong communication, organization, and interpersonal skills
Ability to work effectively with individuals from diverse cultural, socioeconomic, and educational backgrounds
Basic computer skills, including Microsoft Office
Preferred:
Bilingual in Spanish, Fremont, or Somali
Experience working in community outreach, healthcare, or social services
Primarily office and community-based work
May require extended periods of sitting, as well as occasional standing, bending, and lifting (up to 35 lbs.)
Use of standard office equipment such as computers, phones, and copiers
